Background: The role of radiotherapy in the management of inoperable pancreatic cancer continues to be nebulous. SBRT has emerged as an attractive treatment option due to its short treatment duration, minimal chemotherapy disruption, and acceptable side effect profile. Here e report the clinical outcomes from a large single institutional cohort treated definitively ith SBRT. Methods: This retrospective analysis examines 75 patients ho ere all pathologically diagnosed ith pancreatic adenocarcinoma ho ere ineligible for resection. Inoperability as defined as those patients ith borderline resectability ho did not undergo surgery, locally advanced disease, medically inoperability, or those ho refused surgery. SBRT as delivered in 5 fractions to a total dose of 25 to 30 Gy. Chemotherapy as concurrently administered if it as given ithin one eek of the start of SBRT treatment. Toxicities ere scored using the Common Terminology Criteria for Adverse Events version 3. Local control as determined ith radiological follo up utilizing RECIST criteria. Both local control and overall survival ere calculated using the Kaplan-Meier method. Results: A total of 75 patients ith a median age of 69 years 45 to 90 ere treated at Georgeton University Hospital from January 2009 to May 2015. Median pre-treatment ECOG as 1 0 to 3 and the majority of patients ere female 53 percent . The majority of tumors ere localized to the pancreatic head 81 percent and the most common reason for inoperability as locally advanced disease 75 percent . Concurrent chemotherapy as delivered in 55 73 percent patients. FOLFOX and gemcitabine ere the most common concurrent systemic agents utilized in 47 percent and 40 percent of cases, respectively. Fifteen patients ere excluded from RECIST local control analysis due to lack of radiological follo up. Kaplan-Meier 6- and 12-month local control as 88 percent and 72 percent, respectively. Kaplan-Meier 1-year and median survival as 55 percent and 12.26 months, respectively. Conclusions: Five-fraction SBRT can be practically delivered in concert ith chemotherapy and achieves excellent local control in this cohort of patients ith inoperable pancreatic cancer.,J Clin Oncol 34, 2016 suppl; abstr e15670 ,Publication Only Gastrointestinal Noncolorectal Cancer
