Maternal hyponatraemia can also be reflected in the fetus, and some studies have reported increased incidence of respiratory distress and hyperbilirubinaemia in hyponatraemic infants.4,5 In recognition of these studies, changes of obstetric practice were proposed, including administration of oxytocin in sodium-containing fluids.1,6 Many delivery suites restrict oral intake during labour as a preventive measure, believed to reduce the risk of pulmonary aspiration should general anaesthesia become necessary. The question of optimal fluid administration has been addressed in studies indicating that higher volumes of intravenous fluid during labour have positive effects on the progress of labour.7,8 No conclusive results have been published regarding energy requirements during labour.9 In absence of scientific evidence, advice to pregnant women is based on beliefs and poorly founded recommendations.
