Conclusions Renal involvement is a well-documented complication of MM but remains an uncommon sole presentation of the disease. When evaluating the possible causes of acute kidney failure, after excluding pre and post-renal etiologies, MM should be included and pursued as an important possible diagnosis. These patients have poor survival and, therefore, it is of crucial importance to have a high index of suspicion to do an early diagnosis and to promote early and aggressive management. Despite being an invasive procedure, kidney biopsy remains a very significant tool for the differential diagnosis that should not be dismissed.
