• COVID-19 has been associated with cardiac involvement. SARS-CoV-2 requires binding to ACE2 in the RAS. • The ACE2/Ang1-7/Mas pathway counterbalances the RAS, which results in activation of anti-inflammatory pathways. • ACE inhibitors, ARBs, and MRAs upregulate ACE2 activity and expression. • More data are required to determine if regulation of ACE2 in patients with cardiovascular disease and COVID-19 would help improve clinical outcomes.
